The Alzheimer’s Disease Caregiver’s Questionnaire (ADCQ) is a brief, inexpensive screening questionnaire. It is completed by a caregiver (i.e., family member, spouse, companion) or someone else who has knowledge of an individual who is suspected of having Alzheimer’s disease. The instrument is intended to be used to rate only individuals 40 years of age or older. Most people can complete the 18-item ADCQ in 5 to 10 minutes. The cost of processing the ADCQ responses and generating a feedback report is $9.95.

The ADCQ can provide important information that may help you decide what steps to take in determining whether the individual has Alzheimer’s disease. Although the diagnosis of Alzheimer’s disease should not be based solely on the ADCQ results, research has demonstrated that certain patterns of ADCQ answers can identify individuals who later have been diagnosed with probable Alzheimer’s disease. The ADCQ results can indicate the likelihood that the rated individual has a brain disorder suggestive of Alzheimer’s disease.

ADCQ results may also be helpful in convincing an individual that he or she is in need of a professional evaluation. If the ADCQ results suggest that it is likely that the person who was rated has a brain disorder characteristic of Alzheimer’s disease, it is important to obtain a formal evaluation conducted by a qualified health care professional. Professional evaluations for Alzheimer’s disease can be expensive and time-consuming, and many people are reluctant to seek such evaluations until their symptoms are severe. However, the early recognition and treatment of Alzheimer’s disease may delay functional impairment and will allow the individual and his or her family member(s) to make appropriate financial, legal, and residential arrangements under the most optimal conditions. ADCQ results are an inexpensive source of information that can point a caregiver in the right direction.

The results of a completed ADCQ are presented in the Caregiver’s Report, a brief, printable report (click here to see a sample report). The ADCQ Caregiver’s Report may be helpful in summarizing your observations of the individual who was rated. This report can be given directly to the individual’s health care professional.
